Ordering your forms is simple and can be completed in just two easy steps.

1. Payment is quick and easy with our Paypal integration. Simply select which form(s) you would like to purchase, using our simple shopping cart system. Payments are securely handled by Paypal. (all major credit cards are accepted)

2. Send us an e-mail (info@Med-e-Forms.com) containing the following items:

  • Your Company Name, Address and Phone Number
  • E-mail us your Company Logo (if applicable) so we can add it to your Med-e-Forms.
  • Please let us know which e-mail address to assign to the e-mail or Submit button on the Med-e-Form (this is the one in which patients will e-mail the completed form to you.) Only one e-mail address per Med-e-Form is allowed
  • If you need additional e-mail accounts please inquire about pricing.

Once we receive payment confirmation and all necessary information for your Med-e-Form, your completed Med-e-Form(s) will be e-mailed to you within 1 to 3 business days. (Working times for customized Med-e-Forms may vary)
